Class Registration Form

To Register:

Fill out the following form and mail it to Fired Up along with your payment (checks made out to Fired Up, Inc.) Please do not send cash. We will call you to confirm your place in the class.

Please fill out a separate registration form for each person taking the class.

The following requirements apply to ALL classes at Fired Up Studios:

• All classes are adults only (18+)
• The class cost must be paid in full when you register
• If you cancel 1 week prior to the 1st class session, you will be refunded for the class fee minus a $50 administration fee. No refund will be given after the start of the 1st class session.
• Wear clothes that you can get dirty, this includes shoes.

Note: Class schedules and pricing are subject to change; this includes change of instructor, cancellation of classes, and all pricing increases.
